Click on "I ACCEPT" or "I DECLINE" If you accept, you may then download the spreadsheet macro.

After the Excel spreadsheet is downloaded, click on "File" then "Save as"

Save the spreadsheet to an appropriate location on your computer for future use. It can be used repeatedly and indefinitely, for as many patients as desired.

To use this macro, open Microsoft Excel and then open the spreadsheet named "edtadose.xls."

Enter patient data into boxes in the upper left, at the top of column one as indicated, and computation will be automatic.

You may then print out the results for inclusion in the the patient chart.

This Microsoft Excel spreadsheet macro is provided at no cost as a courtesy by Elmer M. Cranton, M.D. and should only be used after the treating physician has assessed each patient individually, including measurement of serum creatinine to assure safety.

Another computation should be done whenever serum creatinine, age, or weight change significantly.

A licensed health care provider should double check all patient data entered into the Excel spreadsheet to insure that the results calculated are safe and correspond to the desired protocol, before that dose is administered to a patient.
